日军入侵中国后，民不聊生，乡下青年陈真（甄子丹 饰）带着妹妹小燕一起来到了繁华的上海讨生活。来到上海后，陈真当了一名黄包车夫，而小燕则进入了一间医院当护士。陈真每天都会拉和小燕在同一间医院当护士的日本姑娘武田由美（万绮雯 饰）上下班，两人渐渐坠入了爱河。陈真爱好武术，闲时经常自己照着书上的招式练习，一次偶然机会他的身手被青帮老大蔡六斤发现，于是被招进了青帮当打手。陈真很快成为了青帮第一打手，而妹妹小燕则一直十分担心哥哥，她始终认为哥哥混黑道不会有好下场的。随后小燕不幸遭到了马贼杀害，悲痛万分的陈真终于醒悟，从此金盆洗手退出了青帮。当时抗日组织精武门的掌门霍元甲发现陈真是一块练武的材料，于是加以指点，得到指点后的陈真武功进步神速，并最终为小燕报了仇。陈真感恩于霍元甲，加入了精武门。这时由美的日本未婚夫来到了上海，当他得知由美和陈真之间的事情后，决心向陈真挑战，胜者就可以抱得美人归。陈真虽然不愿意，但最终接受了挑战。
